The Information You Need:

Copy of student’s birth certificate
Copy of parent’s social security
Copy of parent(s) driver's license
Blue and Yellow medical forms
Withdrawal Slip
School Transcript
Last Report card
IEP or 504 (if available)
Registration fee $75.00

Costs & Subsidies

Now accepting Step-up for students Unique Abilities and PEP.

Our adminission office will provide any information needed and help in completing scholarship applications.
