Enrollment Requirements

The Information You Need:

  • Copy of student’s birth certificate

  • Copy of parent’s social security

  • Copy of parent(s) driver's license

  • Blue and Yellow medical forms

  • Withdrawal Slip

  • School Transcript

  • Last Report card

  • IEP or 504 (if available)

  • Registration fee $75.00​
